Come join us to get vaccinated on Monday, August 23, 2021 from 10:00 am to 12:30 pm at Mt Sinai COGIC, 936 West Ninth Street, Pomona, CA 91766.  We will be offering the Pfizer vaccine for ages 12 and above; and the Johnson & Johnson for ages 18+.  The vaccines are free to all.

Please note the following:

• The Pfizer vaccine will be administered in two doses/appointments
• We cannot give the vaccination to anyone who is currently experiencing flu-like symptoms or is sick.
• For your first appointment, please bring ANY form of identification.
• Anyone under the age of 18 will need a legal guardian present with them and will need to sign a consent form for the child to receive the vaccination.
• A birth certificate will is required for anyone under the age of 18.

After receiving the vaccination, you will sit in the observation area for 15-20 minutes.  Then, you will receive a vaccination card with your second appointment date set for 21 days later, at the same location.
